Who are we?

The Transitions of Care and Primary Care stream is a network of SHPA members who have an interest in strengthening the robustness of the Australian healthcare system in providing continuity of care, including medication management, at discharge from hospital into primary care. Our members may work in hospital-based services or programs including Transition Care, HARP, Hospital in the Home, HOMR or be based in primary care working in General Practice, Community Pharmacy, Home Care Services, Aged Care and/or as independent accredited pharmacists.

Why join?

Join the Transitions of Care and Primary Care Interest Group if you want to access our network of experts and peers through the Transitions of Care and Primary Care Discussion Forum. You don’t need to specialise in transitions of care or primary care to join the Transitions of Care and Primary Care Interest Group.

If you are an experienced primary care or transitions of care pharmacist, why not apply to join the Transitions of Care and Primary Care Practice Group. Belonging to the Transitions of Care and Primary Care Practice Group gives you all the benefits of the Transitions of Care and Primary Care Interest Group, plus opportunities to support members of the Transitions of Care and Primary Care Interest Group and contribute to important SHPA work relating to primary care or transitions of care – including Standard of Practice in Transitions of Care and Primary Care for Pharmacy Services, SHPA submissions relevant to transitions of care or primary care, and SHPA transitions of care or primary care education.

Members of the Transitions of Care and Primary Care Practice Group are also eligible to nominate for election to the Transitions of Care and Primary Care Leadership Committee.
